- FOOD STAMP APPLICATION (FS-1) REVISION
DISCUSSION:
The Food Stamp Application for benefits (FS-1) has been revised. Several changes have been incorporated as a result of feedback from staff, applicants and other stakeholders. The new version includes more spacing, larger font and plain language. The Household Declarations section has been restructured to support staff in completing thorough interactive interviews.
Destroy previous versions of the FS-1 by September 30, 2016 such that the new version is used thereafter.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Destroy/recycle all versions of the FS-1 with a revision prior to September 30, 2016.
- Ensure your office has new copies of the FS-1 before September 30, 2016
- Begin using the revised FS-1 by September 30, 2016
