- DUPLICATE APPLICATIONS FOR MO HEALTHNET FOR AGED, BLIND, DISABLED (MHABD) AND ADULT PROGRAMS
DISCUSSION:
The purpose of this memorandum is to inform staff of the process for handling receipt of a duplicate copy of an MHABD or Adult program application after the original application has been rejected.
FSD must accept a duplicate/copy, or annotated version, of the original IM-1MA submitted as long as the application includes a new signature and date signed. FSD staff must use the date the application is received with the new signature and date, as the new date of application.
Example:
- Mr. Smith applied for MHABD. Mr. Smith failed to provide the requested information timely, and his application was rejected. Mr. Smith resubmitted a copy of the same application FSD originally received with a new signature, and date of request.
- The date the resubmitted IM-1MA was received by FSD is the new date of application for benefits.
It is the expectation of the agency that the applicant or those legally acting on the applicant's behalf will review the original application, and determine if any changes must be made.
NOTE: By signing the original application a second time with the new date of request, the applicant is certifying under penalty of perjury that all declarations made in the eligibility statement remain true, accurate, and complete.
Staff must follow existing AR policy 0130.030.00 DURATION OF APPOINTMENT OF REPRESENTATIVE to determine if the appointment of the AR remains valid.
If the original application is received a second time without a new signature and date of request, staff must follow existing policy on unsigned applications 0105.010.00-0105.015.00 Signing The Application; Dating The Application.
NECESSARY ACTION:
- Accept duplicate/copies, or annotated versions, of MHABD and Adult program applications that include a new signature and date of request, as a new application.
- Review this memorandum with appropriate staff.
