DISCUSSION:
The Visual Disability Examination Report (IM-68) was revised to clarify visual eligibility criteria for Blind Pension (BP) or Supplemental Aid to the Blind (SAB) must be met in both eyes, and to allow the State Ophthalmologist to capture the visual field results on the form.
Effective immediately, use form IM-68 with a revision date of 01/2017 for visual examinations and re-examinations for BP, SAB, or MO HealthNet for the Aged, Blind, Disabled (MHABD) under Blind criteria.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Use the IM-68 with a revision date of 01/2017.
