DISCUSSION:
The MAGI Application for Health Coverage & Help Paying Costs (IM-1SSL) has been revised. Incorporated changes include:
- Application questions follow the application flow for MAGI applications in MEDES
- Rights and Responsibilities page now included so applicants only need to sign in one place
- Updated Appendix C, “Assistance with Completing this Application.”
- Addition of estimated pregnancy due date question
- Address for applicants to send mail-in applications is now listed as follows:

NECESSARY ACTION:
- Review this memorandum with appropriate staff.

- Ensure your office has new copies of the IM1-SSL as soon as possible.
