- REVISED APPOINTING AN AUTHORIZED REPRESENTATIVE (IM-6AR) ADDED TO APPLICATION APPENDIX AVAILABLE ONLINE
DISCUSSION:
This memorandum announces the revised Appointing an Authorized Representative (IM-6AR) form, introduced in Income Maintenance Memorandum #31, has replaced the previous Appointing an Authorized Representative (IM-6AR) form in the applications appendices, available for printing from the Missouri Department of Social Services website.
The revised IM-6AR has also been added to the IM-1MA appendices in the Income Maintenance Forms Manual, on the Department of Social Services intranet and the publicly available Income Maintenance Forms Manual on the internet.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
