FS-61SNAP (FOOD STAMPS) SUMMARY TO DETERMINE FITNESS FOR WORK
DISCUSSION:
The purpose of this memorandum is to inform staff that there is a new, more focused Medical Review Team (MRT) form for Able-Bodied Adults without Dependents (ABAWD) to determine fitness for work for the Food Stamp Program work requirement.
The new form is the FS-61 and can be accessed in the forms manual. This form is to be used to refer a client to MRT only when the Eligibility Specialist (ES) cannot make the determination for unfit for work.

- Review this memorandum with appropriate staff.
