DISCUSSION:
This memorandum is to notify staff of the addition of the FA-402 MO HealthNet Eligibility Review Information form to the IM Forms Manual. FAMIS currently systematically generates the FA-402 annually to active participants of all MO HealthNet Adult programs. FAMIS will continue to generate the FA-402. A hard copy manual version of the FA-402 is being made available to staff for easy access if needed.
The IM-2D Review- Eligibility Statement and the IM-2 MOD Eligibility Reinvestigation forms are now obsolete.
The FA-402 is available in the Forms Manual.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Destroy all IM-2D and IM-2 MOD forms.
