DISCUSSION:
The MO HealthNet Single Streamlined Addition (IM-1ADP) and MO HealthNet Sola Adicion Optimizada (IM-1ADP Spanish) have been revised to follow the flow of the updated IM-1SSL. These are attachments to the IM-1SSL Application for Health Coverage & Help Paying Costs and the IM-1SSL (Spanish) Solicitud de Cobertura de Salud & Ayuda para Pagar los Costos. When an applicant indicates that he/she is applying for more than two people, provide the applicant with IM-1ADP forms for each additional person. For information on how to use the IM-1ADP, refer to memos:

- IM-#116 2013 INTRODUCTION OF SPANISH FAMILY MO HEALTHNET APPLICATION (IM-1SSL SPANISH) AND FAMILY MO HEALTHNET ADD A PERSON (IM-1ADP SPANISH) FORM
NECESSARY ACTION:
- Review this memorandum with the appropriate staff.
- Destroy/recycle all versions of the IM-1ADP/IM-1ADP (Spanish) with a revision prior to 05/17.
- Ensure your office has new copies of the IM-1ADP as soon as possible.
