DISCUSSION:
Policy has been updated to add clarification regarding identity verification for newborn to 16 year old children.
Identity can be verified by the signature of a parent/relative or caretaker/legal guardian on the application or reinvestigation forms for children age newborn up to but not uncluding age 16. This signature is considered an affidavit of the child’s identity.
NOTE: Do not request verification of identity again when the child turns 16 as long as there is continuous eligibility for that child, and there is the prior affidavit signature of the parent/relative caretaker/legal guardian.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
