FORMS MANUAL REVISION #17:  IM-1UA FORM AND INSTRUCTIONS
 
DISCUSSION:
The Children's Health Initiative expands health care coverage to uninsured children with gross family income up to 300% of the Federal Poverty Level (FPL).  Refer to Memorandum #65, dated June 22, 1998.  This memorandum addresses procedures for accepting applications for these uninsured children, beginning July 1, 1998.  A separate memorandum will be issued to address policy and procedures for determining eligibility.

The program name for the new groups of uninsured children is MC+.  To simplify the application process for families, the MC+ program name will now also include the current Medicaid for Children and Medicaid for Pregnant Women (MPW) programs.  Thus, the applicant will apply for MC+ and the worker will register the appropriate application(s) based on the information provided on the revised IM-1UA application form.  If the applicant is pregnant, an MPW application should be registered in IAPP.  If the application is for children, register a Medicaid for Children (C7) application.

Procedures for Accepting MC+ Applications

The IM-1UA will be used to accept MC+ applications.  Systems changes are being made to allow uninsured children in the new eligibility groups to be approved on the C7 case.  Effective July 1, 1998, we will begin accepting applications for these children.  Children eligible in the new coverage groups cannot be approved until the IMU5 system changes are complete.

When an IM-1UA is received, the following procedures apply:

  • Review the IM-1UA to determine the type of assistance requested;
  • Register the appropriate Medicaid for Children (C7) and/or MPW application(s) in IAPP;
  • Determine eligibility and approve children for MC+ if eligible under the current Medicaid for Children income limits;
  • Determine eligibility for children found ineligible for Medicaid for Children by comparing the family's gross income to the income chart (refer to IM #65); and
  • If eligible in a new coverage group, leave the individual application pending until further instructions are received.
REMEMBER:Always determine eligibility for the current family Medicaid programs prior to determining eligibility for the new groups.

IM-1UA Revision

The IM-1UA Application/Eligibility Statement has been revised and renamed MC+ Application/Eligibility Statement.  The current IM-1UA and IM-3MC have been combined into a single form.  It will be used for all MC+ applications for children and pregnant women. 

The revised IM-1UA is used when:  1)  a mail-out MC+ application is requested; 2)  a claimant requests to apply only for an MC+ program while in the office; and 3)  a claimant wishes to make application for MC+ at an outstation site.  The IM-1UA should also be made available at other appropriate locations, such as local hospitals, community centers and schools.  It is important to make sure the local county office address has been stamped on the IM-1UA prior to distributing to other locations, to ensure the applicant will know where to return the completed form.  The current IM-1UA and IM-3MC may be used until the revised form is received.

The following changes have been made to the IM-1UA:

INSTRUCTIONS:

  • The instructions page is perforated to allow the applicant to retain it for their reference.
  • The toll-free number for the phone center referred to in IM-#65 has been added to the instructions page.
APPLICATION PAGE:
  • There are no longer separate boxes to check for Medicaid for Children or Medicaid for Pregnant Women.
HOUSEHOLD MEMBERS:
  • The social security claim number section has been removed.
  • The U.S. citizen section has been removed from the table, and a question concerning citizenship has been added.
  • A question about pregnancy has been added.  If the answer is yes, register an MPW application.
  • A question about net worth has been added.
EMPLOYMENT:
  • This section now asks for the amount of earnings from self-employment.
CHILD CARE:
  • This section asks for the names of the children receiving care.
ABSENT PARENT INFORMATION:
  • A section has been added to give identifying information about any absent parent(s).
  • The applicant is given an opportunity to claim good cause for non-cooperation in obtaining medical support.
HEALTH INSURANCE:
  • This section has been changed to assist the caseworker in identifying families that may be ineligible for MC+ because they do not meet the definition of uninsured.  Being uninsured is only an eligibility factor for the new groups of children, not those that are eligible under the current Medicaid for Children program.
The attached Forms Manual pages are also revised to include these changes.
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.
  • File the revised IM-1UA and instructions in the Forms manual.
  • Begin accepting applications for the new groups of uninsured children on July 1, 1998.
