FORMS MANUAL REVISION #28  IM-2E PART ONE NOTICE OF REQUIREMENT TO COOPERATE AND RIGHT TO CLAIM GOOD CAUSE FOR REFUSAL TO COOPERATE IN CHILD SUPPORT ENFORCEMENT
 
DISCUSSION:
The IM-2E Part One, Notice of Requirement to Cooperate and Right to Claim Good Cause for Refusal to Cooperate in Child Support Enforcement has been revised to include changes in terminology.  The word "recipient" has been changed to participant and "worker" to caseworker/case manager.

There has been no change in policy on how the form is used.  The instructions remain the same.

File the revised IM-2E Part One in the forms manual.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.

    •  
  • Remove the obsolete form IM-2E Part One (11-98) from the IM Forms Manual. Replace it with the revised IM-2E Part One (4-99) form that is attached.

    •  
  • Use IM-2E Part one (11-98) until supply is depleted.
