IM-101  08/11/99  IM-300 FAMILY SELF-SUFFICIENCY PACT

IM-300 FAMILY SELF-SUFFICIENCY PACT FORMS MANUAL REVISION #28 IM-300 and INSTRUCTIONS, PAGES 1 &4
 
DISCUSSION:
The IM-300 Family Self-Sufficiency Pact is being revised to include the Child Protection Clause.  It is also being changed to a carbonless form for the worker's convenience.

The reverse side of the IM-300 informs Temporary Assistance applicants/participants of the Child Protection Clause.  Staff is expected to discuss the Child Protection Clause with Temporary Assistance applicants/participants.  Refer to IM-#157 DETERMINING AVAILABLE, SUITABLE, AND AFFORDABLE CHILD CARE dated December 2, 1998.

The participant's and case manager's signature should be on the original (white) copy and filed in the case record.

Attached is a copy of the revised IM-300 and its revised instructions to be filed in the IM Forms Manual.
 
NECESSARY ACTION:
  • Discuss this memorandum with all appropriate staff.

  • Discuss the Child Protection Clause with Temporary Assistance applicants/participants.

    •  
  • Review the attached IM-300 Family Self-Sufficiency Pact and instructions.

    •  
  • Destroy all copies and instruction for the IM-300, Family Self-Sufficiency Pact dated 2/99.

    •  
  • File the revised IM-300 Family Self-Sufficiency Pact and pages 1 and 4 of the instructions in the forms manual.
