DISTRIBUTION OF SCREENING/IMMUNIZATION BOOKLET
 
DISCUSSION:
The Division of Medical Services has produced the Missouri Healthy Children and Youth Health Screen and Immunization Record.  The purpose of this record is to educate and remind parents and guardians of the importance of having their children immunized and health screened.

In a non-managed care county, please distribute this record to individuals who visit the office and have children receiving or applying for healthcare coverage.  One record should be distributed for each child.  The record should be offered to these individuals when they are in the office for other reasons (i.e. Food Stamps, Temporary Assistance, questions, etc.).  In order to prevent duplication of distribution, inquire of the parent or guardian whether they have already received a record book from another source. 

In managed care counties, the health plans will mail these records to children in their plans.  For children in these counties who will not be enrolled in Managed Care (SSI opted out, MA, etc.), follow the distribution instructions for non-managed care counties.

A supply of record books will be sent to each non-managed and managed care county.  Please call Julie Ousley, Division of Medical Services at 573-751-3277 for ordering additional records when your supply is low.

 
NECESSARY ACTION:
  • Share with all appropriate staff.
  • Begin distribution of the Health Screen and Immunization records to individuals who visit the office.
