REFERRALS TO CONTRACTED CASE MANAGEMENT PROVIDERS
 
DISCUSSION:
The purpose of this memorandum is to clarify which individuals should be referred to the contracted case management providers and information which is to accompany the referral.

TEENAGE PARENTS & TWO PARENT HOUSEHOLDS

Effective immediately, staff should not refer teenage parents or two parent households to the contracted case management providers.  When a teenage parent or a two parent household requires case management, these individuals should be referred to a DFS case manager.

This clarification in policy is necessary as there are different work participation requirements for a single parent and two parent household.  In addition, the case management contract requires a participant to gain full-time employment within 12 months.  This is not consistent with participation requirements for teenage parents.

CLIENT ADDRESSES & TELEPHONE NUMBERS ON THE FP-11

Prior to referring an individual to a contracted case management provider, staff should access the Food Stamp system (FCAS) to check for a current address.  When staff refer an individual to a contracted case management provider, the client's telephone number should be listed on the FP-11, not the DFS telephone number.  The contracted case management providers are to engage the individual as soon as possible after receiving the referral.  When time is spent attempting to contact the DFS worker to obtain current addresses and telephone numbers, there is a delay in the delivery of case management services.

 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.

  • Refer all teenage and two parents households to appropriate DFS staff when case management services are indicated.

  •  
  • Check the Food Stamp screens for a current address for the individual prior to making a referral to a contracted case management provider.

  •  
  • Provide the contracted case management provider with the client's address and telephone number on the FP-11.
