MC+ ADVANCE ACTION NOTICE FORMS REVISION #7:  IM80 MC AND INSTRUCTIONS
 
DISCUSSION:
The IM-80MC has been produced to provide an official notice to MC+ customers of the intent to discontinue or change their level of MC+  health care coverage.  The rest of the instructions provided are self explanatory.  An initial supply of the form is being sent to county offices for use.  Until the forms are available from the warehouse, county offices should make additional copies as needed.
 
NECESSARY ACTION:
  • Share this memorandum with appropriate staff.

  • Begin using the IM-80MC upon receipt.

    •  
  • File the IM-80MC and instructions in the forms manual.
