NAME CHANGES ON EBT CARDS
 
DISCUSSION:
Effective immediately, a new EBT card is required when a name change occurs.  A replacement EBT card must be issued reflecting the name change.

A replacement EBT card may take 4-5 days for receipt.  Be sure the participant understands the option to continue using the EBT card with the previous name until the current month's benefits have been used.  Cash and food stamp benefits cannot be accessed while the participant is waiting for the replacement card.

When the participant reports a name change, discuss when to cancel the EBT card.  Failure to have the new name on the EBT card can cause difficulty when a participant attempts to complete manual food stamp vouchers.  Deluxe is unable to determine whether the current name is the same person shown on the manual food stamp voucher.

Use the following procedures for name changes.

  • Make the necessary changes through SUPD.  This information will update the Administrative Terminal (MOP) overnight.

  •  
  • Discuss with the participant a convenient date to cancel the EBT card.

  •  
  • Verify the address is correct in MOP.  If not, change the address.  Refer to Memorandum IM-#77 (7/9/98) for address change procedures.

  •  
  • On the appropriate date, cancel the card in MOP using A4 (name change) in the card status field.

  •  
  • Access EBTU.  Enter the action code "C" and "Y" in the Reissue Card field.  Press Enter.  You will receive a warning message, "Warning!  Current card has to be canceled in MOP before reissuing card".  Press Enter again.  The message will read, "Record updated.  Enter next DCN".
 
NECESSARY ACTION:
  • Discuss this memorandum with all appropriate staff.

  •  
  • Discuss the appropriate date to cancel the EBT card when the head of household has a name change.

  •  
  • Update SUPD to reflect the name change.

  •  
  • Cancel and reissue a new EBT card reflecting the participant's new name.
