IM-149 AND INSTRUCTIONS, PAGES 1-3
 
DISCUSSION:
Attached is a revised IM-149, Benefit Adjustment Notice.  The revision adds changes in the legal obligation to pay child support to the list of changes that a household must report.

This 3/99 revision of the IM-149 replaces the previous version of the IM-149 and the IM-149(EBT).  Since all participants now receive their benefits through EBT, there is no need for a separate notification form.

Use your current supply of IM-149(EBT) forms to depletion.  The revised form will be sent to offices upon request for a new supply of IM-149(EBT) forms.
 
NECESSARY ACTION:
  • Replace in the IM Forms Manual the previous version of the  IM-149 and the IM-149(EBT) and their instructions with this 3/99 revision and its instructions.

  • Use your current supply of IM-149 forms to depletion.

    •  
  • Begin using the revised form when a new supply of IM-149 forms is requested and received.
