MPW APPR. NOTICE FORMS REV.#2:IM-32MPW & INSTR.
 
DISCUSSION:
The IM-32MPW has been produced to provide women applying for MC+ due to pregnancy (the old Medicaid for Pregnant Women program) an official notice of approval.  The rest of the instructions provided are self-explanatory.  An initial supply of the form is  being sent to county offices for use.  Until the forms are available from the warehouse, county offices should make additional copies as needed.
 
NECESSARY ACTION:
  • Share this memorandum with appropriate staff.
  • Begin using IM-32MPW upon receipt.

  • File the IM-32MPW with instructions in the forms manual.
