MC+ IMPORTANT INFORMATION MC+ INCOME GUIDELINES
FORMS REVISION #12: MC-4 AND INSTRUCTIONS
MC-4A AND INSTRUCTIONS
 
DISCUSSION:
The MC+ Important Information and Income Guidelines have been revised and assigned form numbers.  The revised forms now include information for custodial parents and pregnant women, as well as children, who may be eligible for MC+.  An initial supply of each revised form is being sent to county offices for use.
 
NECESSARY ACTION:
  • Share this memorandum with staff.

    •  
  • Begin using the MC-4 and MC-4A upon receipt.

    •  
  • File the MC-4 and MC-4A and instructions in the forms manual.
