CHANGE REPORT FORM
FORMS MANUAL REVISION #14:  IM-145 FORM AND INSTRUCTIONS
 
DISCUSSION:
The IM-145, Change Report form is revised.  Changes made to the form include:
  • Changing terminology to Temporary Assistance.

    •  
  • Revising the format in Section G to allow households to declare shelter expenses by checking the appropriate blocks rather than filling in blank areas.

    •  
  • Adding in Section I, Child Support Expense, that court ordered child support paid to non-household members includes current payments, arrearages and health insurance as an expense.

    •  
  • Revising Section L to include the current penalties and disqualifications enacted by the Personal Responsibility and Work Opportunity Reconciliation Act of 1996.
 
NECESSARY ACTION:
  • Discuss this memorandum with all appropriate staff.

    •  
  • Use current supply of the IM-145, Change Report form, to depletion.

    •  
  • File the revised IM-145, Change Report form and instructions in the Forms Manual.
