SPANISH MC+ APPLICATION IM-1UA

DISCUSSION:
The MC+ application form, IM-1UA, is now available in Spanish text.  An initial distribution is not being done at the present time.  The form is available at the distribution warehouse for counties that have a significant need for a quantity of this form.  Request form number MO 886-3688 (3-99) IM-1UA (3-99).  The electronic version of the Spanish MC+ application is also available for counties who have only a sporadic need for the translated form.

A supply of the Spanish IM-1UA may be furnished to local community groups who work with the Spanish speaking population.
 
NECESSARY ACTION:
  • Share this memorandum with appropriate staff as soon as possible.

  • Begin using the Spanish IM-1UA for Spanish speaking customers.

    •  
  • File the Spanish version IM-1UA in the Forms Manual behind the English version.
