FORMS MANUAL REVISION #22: IM-30 FORM AND IM-30 FORM INSTRUCTIONS
 
DISCUSSION:
Effective April 1, 1999, the Federal Poverty Level income guidelines increased.

The IM-30 form is revised to reflect the new poverty level figures for the MC+ (for children, custodial parents, pregnant women) and the Transitional Medical Assistance programs.  The 125% poverty level table used to determine eligibility for MC+ non-custodial parents has been added to the chart.
 
NECESSARY ACTION:
  • Review this memorandum with all appropriate staff.

    •  
  • File revised form IM-30 in the Forms Manual.

    •  
  • Begin using the revised IM-30 upon receipt.

    •  
  • Discard obsolete IM-30 budget forms and instructions.
