IMU5 CODE SHEET FORMS MANUAL REVISION #25
 
DISCUSSION:
The IMU5 Code Sheet has been revised to include the codes and fields implemented as the result of changes in Temporary Assistance, MAF, MC+, and Medicaid.  The IMU5 Code Sheet now consists of two pages, one for Individual Data and one for Case Data.

A number of codes have been removed since they are now obsolete.  They are:

Field 7 --  Reason for Rejection or Closing

50 - Receipt of lump sum (AFDC)
57 - Failure to comply with FUTURES requirements
58 - Loss of 30 + 1/3 or $30 disregard by FUTURES participant
Field 8 --  UP Code
W - UP case under 21 qualified by waiver
Field 13G2 --  Sanction Reason/Reason for T-XIX Only
D - Refusal to comply with Direct Job Placement
Field 13G3 --  Sanction Type

Field 13P1 --  Work Requirement Codes

N - Mandatory FUTURES participant who is volunteer for immediate participation
J - Non-FUTURES participant in an approved FUTURES activity applying for or receiving FUTURES Child Care - Mandatory 
K - Non-FUTURES participant in an approved FUTURES activity applying for or receiving FUTURES Child Care - Exempt
Field 18 --  Priority Codes
K - Referral of parent or child for FUTURES
P - Contact ES for unemployed parent (AFDC-UP)
Field 38A -- Extended XIX
W - Employment related closing
D - Removal of disregard
 
NECESSARY ACTION:
  • File the new IMU5 Code Sheet in the Forms Manual.

  • A code sheet for each IM worker is being sent to the county office.

    •  
  • Additional copies of the code sheet may be obtained from the warehouse.

    •  
  • Destroy all obsolete IMU5 Code Sheets.
