MEDICAID AND YOU PAMPHLET  FORMS MANUAL REVISION #26:  IM-4 MEDICAID AND YOU
 
DISCUSSION:
The revised IM-4 Medicaid And You pamphlet is available.  The revision reflects current policy for those individuals receiving Medicaid because they are elderly, blind or disabled.  Use the pamphlet to explain the Medicaid program to applicants and recipients.  The pamphlet is not for distribution to individuals receiving MC+ health care coverage (C types of assistance, MPW, and Refugee Assistance).

The pamphlet includes explanations on:

Spenddown, 
Third party liability, 
Health Insurance Premium Payment(HIPP)  
Covered services, 
Services not covered, 
Exception process, 
Limitations, 
Cost sharing, and 
Personal rights under Medicaid.
The pamphlet provides general information about covered services.  Specific coverage questions should be directed to the person's provider or Division of Medical Services Recipient Services Unit at 1-800-392-2161.

An initial distribution is being sent to each county office.  Use the current supply of the IM-4 Medicaid And You pamphlets until receipt of the revised pamphlet.  Request the revised pamphlets through normal procedures.

  • Review this memorandum with all appropriate staff.
