If a provider reports that a household failed to pay the sliding fee amount, verify the amount owed to the provider is the sliding fee amount, as opposed to a co-payment. If it is documented that the family has, in fact, failed to pay the sliding fee portion, complete an adverse action to close the EU. See FAMIS User Guide – Worker Initiated Closing. If the household pays or signs an agreement with the provider to pay the sliding fee amount, obtain documentation of the payment or agreement and consider the household eligible.
