1102.005.25 Inconsistent Information

When further verification is required to resolve inconsistent information, record the following:

  • why the information is considered inconsistent;
  • why an alternate source of verification, such as reference/collateral verification (CC or TC) or home visit, is needed;
  • the reason a reference/collateral verification (CC or TC) is rejected and the reason an alternate verification is requested; and
  • what verification is being used to resolve the inconsistent information.
