Provide to the claimant the following leaflets:

  1. HCY, if applicable
  2. Important Information, MC-4
  3. Non-Emergency Medical Transportation

Complete the following forms, at a minimum:

  1. Application and Eligibility Statement, IM-1UA
  2. Non-Custodial Parent eligibility for MC+, IM-4UANCP
  3. TPL Form (if necessary)
  4. Referral for Social Security Number, SSN-1 if necessary
  5. Request for Information, MC-31A if needed

Explain the following eligibility requirements:

  1. Social Security Number
  2. Assistance Grouping
  3. Residence
  4. Citizenship
  5. Alien Status
  6. Need
  7. Current in payment of Child Support.
