Acceptable forms of documentation that verify both citizenship and identity are:

• U.S. passport,
• Certificate of Naturalization (N-550 or N-570), and
• Certificate of Citizenship (N-560 or N-561),
• Missouri Driver’s License Inquiry (MODL) system where the Detail Identification Inquiry screen (upper right side) indicates that the person is a verified U.S. citizen.

If one of the above forms is not available, you must obtain one of the following documents to verify citizenship and a second source to verify identity:

• Birth certificates or IBTH (if born in Mo) – do not print IBTH screens or file in the case record,
  • Note: Effective October 1, 2010, a voided Puerto Rican birth certificate is not acceptable verification of citizenship. The participant will need to provide other documentation of citizenship.
• Certification of Birth (DS-1350),
• Consular Report of Birth (FS-240),
• Certificate of Birth Abroad( FS-545),
• U.S. Citizen ID Card ( I-197 or I-179),
• American Indian Card ( I-872),
• NMI card ( I-873),
• Final adoption decree,
• Official Military Record of Service,
• Hospital record on hospital letterhead,
  • Note: For children under 5, must be at or near time of birth
• Life or health insurance record showing a U.S. place of birth created at least five years prior to application,
• U.S. State Vital Statistics official notification of birth registration,
• Statement signed by physician or midwife present at time of birth,
• Institutional admission papers created at least five years prior to application date that includes a U.S. place of birth, and
• Medical (clinic, doctor, or hospital) records that created at least five years before application.

Acceptable forms of documentation that verify identity include:

• Driver’s license issued by the State or U.S. Territory either with a photograph or other identifying information,
• Certificate of degree of Indian blood,
• School identification card,
• U.S. military card or draft card with photograph,
• Identification card issued by the Federal, State, or Local government,
• Military dependent card if it includes a photograph, and
• Native American tribal document.

For children under age 16, identity can be verified by the signature of a parent/related caretaker/legal guardian on the application or reinvestigation forms. This signature is considered an affidavit to the child’s identity.

NOTE: Do not request verification of identity again when the child turns 16 years old as long as there is continuous eligibility for that child and there is the prior affidavit signature of the  parent/related caretaker/legal guardian.

For all other individuals, including children applying as their own caretaker, the following documents can be used to verify identity:

• School records that include the date and place of birth and parent(s) name (School records may include nursery or daycare records),
• Clinic, doctor, or hospital record showing date of birth,
• Court orders, and
• Verification of G LOC newborn coverage (also serves to verify citizenship).

If the individual is unable to obtain documentation listed above for either citizenship or identity, contact the Income Maintenance Program and Policy Unit for further assistance.

To enter non-citizenship information in FAMIS, complete the following steps.

• Go to the Non-Citizen Information (FMML) screen;
• Prompt for the EU member if not displayed;
• Enter the date of entry, status, and status begin date;
• Enter NAT (Nationals) in the status code filed.

If verification is not provided, leave the document type and verification fields blank. FAMIS pends the application until proper documentation is provided and entered. The applicant’s statement that he or she is a U.S. non-citizen National is sufficient for interim eligibility. Enter CS (client statement) if verification is not provided. At the next certification, remove the CS code and verify the status.
