FAMIS generates renewal applications to providers 60 days prior to the end of the current registration period. FOL and OPU providers receive the Provider Registration Renewal Background Screening Request” (FA 159) as a part of this renewal packet. FOL & OPU providers must complete the FA 159 listing the name, date of birth and SSN for themselves and all household members age seventeen and older and submit to CCPRU or the local FSD office. Staff must determine if the individuals listed on the form are registered with the FCSR by checking the FCSR web site as outlined in section 1215.025.20.05.05. If all individuals listed on the form are registered with the FCSR, request a current background check at http://www.dhss.mo.gov/FCSR/.
