If the verification received contains enough information for a reasonable person to determine the work schedule (wages and schedule/hours worked), an authorization can be entered without having to request additional verification of work schedule A detailed comment should be made on the Income (FMX0) screen in FAMIS. If employment verification is not sufficient to support an authorization to a provider in FAMIS, the CD202 Child Care Schedule Verification form can be used to verify work schedule. For self-employed individuals: If the business has been in operation for more than a year, obtain a copy of the individual’s previous year’s tax return, including all schedules. If the business has not been in operation more than a year, obtain a copy of the individual’s business records. These business records must show all income and expenses, as well as the dates received and incurred.
