SUBJECT:
CHILD CARE ASSISTANCE PROGRAM BROCHURE (IM-4CC)
 
DISCUSSION:
The Child Care Assistance Program brochure (IM-4CC) has been revised.  Destroy/recycle old brochures upon receipt of the revised brochures.

The brochure may be distributed to both clients and child care providers to assist them in understanding the application process, eligibility determination, child care provider selection, and payment procedures.

A sample of the IM-4CC is attached to this memorandum.  They are available from the warehouse following normal ordering procedures.
 
NECESSARY ACTION:
  • Share this memorandum with appropriate staff.
  • Destroy/recycle the IM-4CC with a revision date of 9/92.
  • File the revised IM-4CC and instructions in the IM Forms Manual.
  • Order additional supplies of the Child Care Assistance Program (IM-4CC) brochures from the warehouse using normal ordering procedures.
