IM-115, REQUEST FOR FOOD STAMP HOUSEHOLD REPORT AND INSTRUCTIONS

DISCUSSION:
Effective immediately, it is no longer necessary to complete the hard copy of form IM-115, Request for Food Stamp Household Report.  The IM-115 is available on E-mail and should be used. 

Requests for a food stamp household report can be sent to FAMIS at F051IDM.  Processed requests will be returned to the county office

Attached to this memorandum are the online instructions.  Please file the instructions in the IM Forms Manual.

Staff may use the hard copy of the IM-115 until depletion.  Once the current supply is exhausted, the form will not be available at the warehouse.
 
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  • Send requests for food stamp household reports to FAMIS at F051IDM.
  • Use the current supply of form IM-115 to depletion.
