Effective immediately, all completed DESE-6
forms will be sent to Central Office to the attention of Debbie Bower-Hazlett,
instead of to DESE. Any DESE-6 forms received by DESE will be forwarded
to Central Office for review.
The DESE-6 form refers an individual for Vocational/Job Skills Training funding or revises the initial request due to changes in circumstances. Vocational/Job Skills Training funding is applied to DESE certified courses only. Participants shall only be enrolled in training up to and including the Associates Degree level. Do not use Vocational/Job Skills Training funds to pay for advanced degrees beyond Associates Degree level. Do not authorize DESE training funds for Certified Nurse Technician (CNA). Many hospitals and nursing homes provide this training when the participant is employed with their institution. Reimbursement for training costs (tuition, fees and supplies) shall not exceed $3500 per participant per year for a maximum of two years. Participants requesting funding must apply for a PELL grant prior to completing the DESE-6. PELL grant funds must be used prior to utilizing Vocational/Job Skills Training funding. All referrals must contain the signature of a DFS case manager and the approval of local supervisory staff. A written authorization from the local DFS Welfare Reform Coordinators must accompany the DESE-6 referral form on all exceptions. Refer to the Case Management Manual, 0285.005.15 through 0285.005.15.20 for further information. Participants can be in an education activity up to their 60 month time limit as long as they meet their required hours. |
