FORMS MANUAL REV. #34, IM-2E (PART 1), NOTICE OF REQUIREMENT TO COOPERATE AND RIGHT TO CLAIM GOOD CAUSE FOR REFUSAL TO COOPERATE IN CHILD SUPPORT ENFORCEMENT
 
DISCUSSION:
Wording on the IM-2E (Part 1), Notice of Requirement to Cooperate and Right to Claim Good Cause for Refusal to Cooperate in Child Support Enforcement, has been revised to reflect healthcare coverage instead of Medicaid.  The IM-2E (part 1) also now advises the claimant their children may still be eligible for healthcare coverage for refusal to cooperate in obtaining medical support without "good cause" but they will not.

There has been no change in policy for use of this form.  This form is only required for Temporary Assistance applications.   The form is not required if applying for MAF or MC+ only.
 
NECESSARY ACTION:
  •  Review this memorandum with appropriate staff.
  • Destroy the current supply of IM-2E (Part 1) upon receipt of an initial supply.
  • File the revised form in the Forms Manual.
