The forms used to register child care
providers have been revised to reflect changes as a result of RSMo 210.010
and rate enhancements. These changes include requirements for background
screenings, tuberculosis testing, minimum age and notification of access
to a telephone.
Destroy/recycle any child care vendor registration forms with revisions dated prior to 1999 upon receipt of the revised forms. A copy of each revised form and instructions are attached. All revised forms can be ordered from the warehouse using normal ordering procedures. These forms are to be used for new providers and providers whose registration has lapsed in FAMIS. Providers who are renewing their registration are to use the FAMIS system generated forms. |
