MEDICAL REFERRAL AND AUTHORIZATION LETTER FOR EMOTIONAL HARM DETERMINATION (IM-60C); FORMS MANUAL REVISION #33
DISCUSSION:
The purpose of this memorandum is to inform staff that the Referral Form-Summer Employment (IM-51A) and the Medical Referral and Authorization Letter for Emotional Harm Determination (IM-60C) forms are now obsolete.  All remaining copies in the counties should be destroyed.

Follow regular procedures for good cause determination in accessing emotional harm to the parent or child from cooperation with the Division of Child Support Enforcement.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  • Discontinue the use of the IM-51A and the IM-60C.
  • Make pen and ink changes to the Forms Manual to obsolete these two forms.
