FORMS MANUAL REVISION #18: FORM IM-320, WAGE SUPPLEMENTATION PARTICIPATION AGREEMENT
DISCUSSION:
The form IM-320 Wage Supplementation Participation Agreement is revised to reflect the change of removing food stamps from the diversion to the employer.  The diversion amount is now $247.00. 

No changes have been made to the form's instructions.

The revised form is being printed and upon completion, an initial supply will be sent to county offices.  Upon receipt of the revised IM-320, destroy all former revisions.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  • Destroy all former revisions of the IM-320 upon receipt of an initial supply of the revised form.
  • File the revised IM-320 form in the IM Forms Manual.
