DISCUSSION:
The Domestic Violence Screening (IM-2DVS) form has been updated to include the following changes:

FIRST PARAGRAPH

This statement has been added:

  • Your responses will not effect your eligibility for this program.
SECTION 1
  • The questions have been arranged to indicate order of importance.
NECESSARY ACTION:
  • Discuss this memorandum with appropriate staff.
  • Update the Forms Manual with the 04/02 revision of the IM-2DVS instructions. 
  • Use existing supply to completion.
