FORMS MANUAL REVISION #3:  IMU5 CODE SHEET
DISCUSSION:
The IMU5 Code Sheet has been revised.  It has been updated to include the following changes:
  • Field 5 - Case Actions
    • 06 - Reopen MC+/T-XIX
    • 22 - Reduce due to WIN employment has been removed
    • 60 - State Office Action has been added
  • Field 7 - Reason for Rejection or Closing
    • 49 - Removal of 2/3's, 30 + 1/3 or $30 (2/3's has been added)
    • 56 - UP working 100 hours or more has been removed
    • 67 - Treatment Terminated has been added
    • 68 - (SO only) System Closing End of Time Limit
  • Field 8 - Changed from UP Code to Two Parent Code
    • U - Changed from Unemployed Parent Case to Two Parent Household Case
  • Field 13C- Race
    • 1 - White
    • 2 - Black/African American
    • 4 - American Indian/Alaskan Native
    • 5 - Asian
    • 6 - Native Hawaiian/Pacific Islander
    • 7 - Multi-Racial (computer generated when two codes entered)
    • U - Unable to determine
  • Field 13F - Unrelated Guardian (The following GAFP codes have been added)
    • G - Grandparent under 50-has training, has custody 
    • A - Grandparent under 50-has not has training, does not have custody 
    • B - Grandparent under 50-has had training, does not have custody 
    • C - Grandparent under 50-has not had training, has custody 
    • F - Grandparent over 50-has training, has custody 
    • D - Grandparent over 50-has not had training, does not have custody 
    • E - Grandparent over 50-refuses training, has custody 
    • H - Grandparent over 50-refuses training, does not have custody 
    • I - Grandparent over 50-has training, does not have custody 
    • J - Grandparent over 50-has not has training, has custody 
    • K - Other relative over 50-has training, has custody
    • L - Other relative  over 50-has training, does not have custody
    • M - Other relative over 50-has not had training, has custody
    • Q - Other relative over 50-has not had training, does not have custody
  • Field 13G2 - Level of Care
    • L - BCCT Presumptive Eligibility has been added
    • W - BCCT has been added
  • Field 13K - Alien Status 
    • F - American Indian Born in Canada has been added
    • H - Trafficking Victim has been added
  • Field 13M4 - Student/Training Status 
    • D - ABE program has been changed to AEL program
    • I - JTPA has been removed
  • Field 13P1 - Work Requirement Codes
    • A - Non-Custodial Parent (Parent's Fair Share computer generated)
  • Field 13P2 - Exempt Reasons
    • 02 - Custodial caretaker of a child under 12 months of age for MORE than 12 months has been added
  • Field 13Q - UP has been changed to Two Parent Household
    • U - Unemployed Parent has been changed to Two Parent Household
  • Field 23 - MA-NC Eligibility Criteria
    • L - BCCT-PE has been added
    • W - BCCT has been added
  • Field 27 - Case Identifier
    • G - Grandparents as Guardians has been removed
    • L - Lutz case has been removed
    • T - TXIX, Not Deprived of Parental Support has been removed
This code sheet will be available on the Intranet Site only.  It will not be available to counties in printed form.  To access the code sheet, from the DFS Intranet select Code Sheets under the Tools section.

To print the new code sheet, choose IM Code Sheets from the DFS Index page.  Choose IMU5.  Choose printable version link.
NECESSARY ACTION:
  • Discuss this memorandum with appropriate staff.
  • Destroy the 04/99 code sheet.
  • Begin using 1/02 version of code sheet immediately.
