FORMS MANUAL REVISION #2:  APPLICATION FOR BENEFITS (IM-1) REVISION
DISCUSSION:
The Application for Benefits (IM-1) form has been updated.  It includes the following changes:

Above the DOUBLE LINES (in the center of the page) the following categories have been added:

  • BCCT and NEWBORN have been added to the types of assistance an individual can apply for.
Below the DOUBLE LINES (on the lower right hand side of the form) the following categories have been removed:
  • NEWBORN
  • APPLICANT NOT RECEIVING TEMPORARY ASSISTANCE
  • TEMPORARY ASSISTANCE-UP APPLICATION
  • CHANGE OF PAYEE 
  • OPEN CASE (ADDING TEMPORARY ASSISTANCE-UP PARENT
Updated forms instructions for the 10/01 revision are included in this memorandum.
NECESSARY ACTION:
  • Discuss this memorandum with appropriate staff.
  • Begin using the 10/01 revision of the IM-1 after the current stock has been depleted.
  • Update the Forms Manual with the 10/01 revision of the IM-1 and the 10/01 revision of the IM-1 instructions.
