DISCUSSION:
Income Maintenance Memorandum IM-#83 dated July 3, 2002 referenced new forms to be used in the evaluation process for extending a family's 60-month lifetime limit due to a hardship.

The following forms and instructions are attached:

  • 60-Month Review Checklist (IM-360)
  • Extension or Closing Summary (IM-360A)
  • Letter of Invitation (IM-362)
The forms will be sent to the county office in an initial distribution.  Forms' templates are available to download.

The Notice of Temporary Assistance Extension for Hardship Action (IM-363) and the form's instruction will be sent in a separate memorandum and distribution.  Do not use IM-363 with revision date of 05-02 received in training.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff.
  • Review IM-#83 for use of the forms.
  • File the forms and instructions in the Forms Manual.
  • Add the above forms to the Forms Manual Table of Contents by pen and ink changes.
  • Immediately begin using the new forms to evaluate individuals who have reached their lifetime limit.
