FORMS MANUAL REVISION #1:  REPORT TO FAMILY SERVICES ON REFERRALS OF ADC PERSONS (IM-52)
DISCUSSION:
The purpose of this memorandum is to inform staff that the Report to Family Services on Referrals of ADC Persons (IM-52) is now obsolete.  All remaining copies in the counties should be destroyed.
NECESSARY ACTION:
  • Review this memorandum with appropriate staff. 
  • Discontinue the use of the IM-52.
  • Make pen and ink changes to the Forms Manual to obsolete this  form.
