As discussed in Memorandum IM-#65 dated 6/29/04, when an EU requests a hearing based on a food stamp METP sanction, a copy of the Application for State Hearing (IM-87) is faxed to the Division of Workforce Development (DWD). Since the IM-87 does not always contain the name of the individual EU member who is sanctioned or is being sanctioned, immediately begin including the name and Social Security number for the sanctioned individual on the FAX cover sheet when sending to DWD administration office at (573) 751-9528.
Currently, DWD is not able to identify which individual EU member failed to comply when the sanctioned individual is not the head of EU. Providing the name and Social Security number for the sanctioned individual on the FAX cover sheet allows DWD to identify the EU member being sanctioned and prepare testimony and/or documentation for the hearing prior to the hearing date.
