DISCUSSION:
Form FS-1, Applicant's Eligibility Statement, English and Spanish translations, are revised. A statement is added to notify the EU that providing the race/sex of an individual is voluntary and does not affect his/her eligibility or benefit amount for the Food Stamp Program. This revision does not require changes to the FS-1 instructions.
The revised FS-1, English and Spanish translation, are available on the Intranet and available on the Department of Social Services web page.
An initial supply of the revised FS-1 is being sent to the county offices. Upon receipt, destroy/recycle all versions prior to 04/05.
NECESSARY ACTION:
- Review this memorandum with all appropriate staff.
