DISCUSSION:
Form IM-23, Client Services Postcard and instructions are revised to remove references to Division of Family Services, AFDC, and Caseworker to read as Family Support Division, Temporary Assistance, and Eligibility Specialist. The Post Office Box has also been changed so that the cards come directly to the Family Support Division.
An initial distribution is being sent to each county office. Upon receipt of the revised form, destroy/recycle all versions with a revision date prior to 04/06.
The form and instructions are available on the Intranet for review.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Upon receipt of the revised IM-23, destroy/recycle all versions previous to 04/06.
