IM-4 FRAUD PAMPHLET
DISCUSSION:
A new pamphlet has been created. The IM-4 Fraud pamphlet provides information pertaining to the following topics:
- What is fraud?
- What are the fraud penalties?
Restitution;
Criminal penalties;
and Disqualification. - Why fight fraud?
- Help fight fraud
- How can I report fraud?
This pamphlet is to be provided to IM Medicaid and Temporary Assistance families at application, during the face-to-face interview, when completing a reinvestigation, generating county office letters for Medicaid or Temporary Assistance Programs, and when mailing any other IM correspondence to the family. The pamphlet may also be placed in the reception areas in the county office. This pamphlet is NOT to be used for the Food Stamp Program.
An initial mailing of the IM-4 Fraud pamphlet is being completed at this time. Use normal ordering procedures to obtain a supply of this pamphlet from the warehouse. The IM-4 Fraud pamphlet 
, with instructions, is also available via Department of Social Services Intranet in the Forms Manual, Volume I.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Provide the pamphlet to the EU as described above.
