FOOD STAMP APPLICATIONS MADE OUT OF THE OFFICE DUE TO MARCH 2006 STORMS
DISCUSSION:
Due to the severe storms and tornadoes in Missouri March 11-13, 2006, several counties were declared disaster areas. Some Family Support Division county offices are participating in disaster sites, taking food stamp applications and assisting as needed.
Staff who are taking food stamp and/or Temporary Assistance applications and interviewing the EU away from an FSD office may not be able to access FAMIS to complete an interview. An EU may not be able to come to the office, send a representative, or have access to a telephone for the interactive interview.
To aid staff in completing an application interview for food stamps and/or Temporary Assistance, attached to this memorandum is a guide that includes the questions in FAMIS for food stamps/Temporary Assistance programs.
- For food stamp applications, ask the applicant to complete the FS-1, then use the Interview Guide to complete the interview.
- For Temporary Assistance applications, complete an IM-1 and use the Interview Guide to complete the interview.
Upon return to the FSD office, enter the information from the interview guide into FAMIS. Attach the interview guide to the food stamp or Temporary Assistance application completed by the EU.
This guide is to be used until the disaster sites are closed down and applications can be taken in the FSD county office.
