Forms IM-2A 
, Blind Pension Supplement (04/06) and IM-2B , Statement of Parent or Sighted Spouse (05/06), with instructions (IM-2A, IM-2B), are revised to remove Division of Family Services and change it to read Family Support Division.
These forms are available on the Intranet in the IM Forms Manual, Volume 1 and are available as Word documents. These forms are not being printed. Destroy/recycle all previous versions of the IM-2A and IM-2B.
NECESSARY ACTION:
- Review this memorandum with appropriate staff.
- Destroy/recycle all previous versions of the IM-2A and IM-2B.
