DISCUSSION:
Forms IM-1U 
, MC+ Annual Review, and IM-1UA , Missouri MC+ Application, are revised. These forms are available on the Intranet in the IM Forms Manual, Volume 1.
An initial shipment of the revised forms is being printed and shipped to each county office. Until the initial shipment is received in the county office, download the forms from the Intranet. Immediately destroy/recycle the IM-1U and IM-1UA with a revision date prior to 07/06.
These forms will be available in d:/fsforms soon.
