FORMS MANUAL REVISION #15: IM-39, REQUEST FOR EMPLOYMENT SECURITY INFORMATION – OUTSIDE THE STATE OF MISSOURI AND INSTRUCTIONS
DISCUSSION:
Form IM-39, Request for Employment Security Information – Outside the State of Missouri, and instructions, which was previously obsolete, has been revised and reinstated and is used for non-FAMIS programs. Use the IM-39 to inquire about an applicant or recipient’s receipt of wages or unemployment insurance from another state. The form may be used at application or reinvestigation, or when information is received indicating that an individual works or receives unemployment insurance from another state. For FAMIS programs, use form FA-327.
Use of the IM-39 would be appropriate if the claimant or an assistance group member:
- reports wages or unemployment insurance in another state;
- has a history of employment in another state;
- recently moved to Missouri from another state and there is reason to believe s/he may be employed or receiving unemployment insurance in that state; or
- lives in close proximity to another state and there is reason to believe s/he may be employed or receiving unemployment insurance in that state.
Changes to the form are as follows:
- The form is now intended to be sent by the eligibility specialist to the receiving state.
Previous instructions to send the form to FAMIS are obsolete.
- The body of the form:
- allows the eligibility specialist to indicate the reason the information is being requested, and
- specifies the information needed from the receiving state.
- An Authorization for Release of Information section has been incorporated into the form. It is no longer necessary to attach an IM-6.
Use the online version of the IM-39 as this form is not being printed. The IM-39 form and instructions can be found on the Family Support Division Intranet in the Forms Manual, Volume 1. Scroll down to the IM-39 and either select the form or instructions. The word version will be available soon.
NECESSARY ACTION:
- Review this memorandum with all appropriate staff.
- Begin using Form IM-39 immediately for cases that are not in FAMIS.
- Continue to use the FA-327 for FAMIS cases, to allow FAMIS to track outstanding verification.
